I would agree that American policies have increased the size of our healthcare sector, but is this really something that can be attributed to free market decisions? How much more growth can we handle when it is already 18% of our GDP ($3.4 trillion)? There is probably an argument to be made that healthcare in the USA is better than a place like Canada, but I really don't think it is 2 times better (~$10,000 vs ~$5,000 per capita).

1. The USA has the best doctors and hospitals. The marginal benefit to having better doctors and hospitals is completely dwarfed by their higher prices. Would you be willing to pay 2x to have a 5% better chance of no complications on a surgery for a rare condition? Well the USA does exactly that.

2. The USA subsidizes drug development for the rest of the world. All the R&D happens here, and the only reason so much money is spent on this R&D is because the drug companies can charge maximum price to Americans. Other countries negotiate as a block to lower prices. This is great for those countries (cheaper drugs) and the drug companies (marginally expanded markets), but it increases the fraction of money from the US going to that R&D.

It doesn't, what he is saying is that the companies that do crash and burn after revealing salary information to their employees are the ones that should crash and burn. There is not an inherent evil in paying a lower salary. On the other hand,if the only reason they are able to pay a lower salary is because their employees do not know any better, they are more likely to be negatively affected by opening their salary information.
