thenerdhead··on Scientists discover a cause of lupus, possible way to reverse it
Very likely. All of these illnesses are diseases of the cells. We're entering the golden age of immune cell science. You figure out what immune cells are causing disease and how to restore them.

Here the T cells are imbalanced and a specific protein is found to regulate the imbalance but the interferon is countering the protein's effects. Now you can target that in many ways and run all sorts of clinical trials.

thenerdhead··on Scientists discover a cause of lupus, possible way to reverse it
Many ME experts are doing both. RECOVER is considering adding ME/CFS arms to its huge clinical trial platform. UCSF just added ME/CFS as a priority in their LIINC program.

SARS-CoV-2 therapeutics won’t work on both but immune cell based ones may given they haven’t been tested in either yet.

Both suggest a root cause of persistent viral antigen. Time will tell what works here.

thenerdhead··on Will we die from minor scratches in the future? Q&A antibiotic resistance expert
Where is the accountability from health experts? Shifting the blame to patients is ridiculous and irresponsible.

Humans naturally have diverse immune systems to prevent our species from being wiped out by a single pathogen.

We should invest much more money into countermeasures and advancing our understanding of complex systems like the immune system and the microbiome.

AI is crucial in this area nowadays. New therapeutics countering resistance are emerging. We should double down.

thenerdhead··on Dr. Vivek Murthy: Social media is a key driver of our youth mental health crisis
Funny how every social media company launches commercials saying they will cooperate with congress once they got the spotlight on this issue.

This has been a problem for decades and I personally was in denial until I realized how much anxiety, depression, etc these things added to me daily from doomscrolling. I found a new love for nature and books after taking a serious look at my own mental health as a millennial who used social media as it became big.

thenerdhead··on Dr. Vivek Murthy: Social media is a key driver of our youth mental health crisis
typical comment blaming the parents instead of the decades of innovation leading to addictive technology behaviors.
thenerdhead··on Social Media Messed Up Our Kids. Now It Is Making Us Ungovernable
I agree. There have long been concerns about the impact of various media on children, as highlighted by figures like Jerry Mander, Neil Postman, and even Ron Serling.

The issue lies not only in the quality of the content but also in the mediums through which they are consumed.

Today, we are surrounded by low-quality content that can be quickly and easily accessed. Those familiar with ADHD(and struggle with it) understand that the instant gratification from these videos can lead to a craving for more.

As a millennial who grew up with unrestricted access to the internet and the advent of smartphones, I truly believe the situation will continue to worsen rather than improve.

I even wrote an entire book on this topic, detailing how internet addiction has profoundly impacted my life.

thenerdhead··on A primer on the next generation of antibodies
> Potentially, antibodies, as a functional category of therapeutic, may very well be on their way out.

I’m a bit confused as to what you reference here but I’m assuming monoclonal (mAbs). I apologize I did not read your prior post. The market estimates are quite optimistic in this category such as:

> Antibodies are the dominant class of protein therapeutics with over 160 antibody therapeutics currently licensed globally and a market value expected to reach $445 billion in the next five years

https://www.ncbi.nlm.nih.gov/pmc/articles/PMC10983868/

Are you aware that BARDA is investing $5 billion into therapeutics like monoclonals/polyclonal for COVID-19 countermeasures? Their major focus is on many of the problems you stated and they want to drive costs down:

https://youtu.be/t9lrPDxLsfw?si=T87jPBinbuTtDad_

Monoclonal antibodies seem like they finally are reaching the golden age based on their use earlier this pandemic, novel findings for rejuvenating our immune system, and even targeting other notable chronic diseases.

https://medicalcountermeasures.gov/nextgen/

https://www.nih.gov/news-events/nih-research-matters/rejuven...

https://www.nih.gov/news-events/news-releases/nih-scientists...

The primer in the next generation is wonderful. I’d love to see nanobodies created for SCV2 as soon as possible. The interesting part is that these platforms that BARDA are helping get online will help us pivot to any threats and use these next generation vehicles to respond. Keep an eye on vaccines too as they are going through a moment of next generation vehicles as well. I.e. multiple antigens, mucosal deliver, etc.

thenerdhead··on Influencer cartels manipulate social media
I feel like this has existed for all of the early influencers who continue to peddle their mediocre NYT mostly self-help bestsellers. Probably even to name a name, something that Ryan Holiday infamously wrote about early in his publishing career.

I knew a person who did some type of manipulation for their TikTok up to 550k followers in 2020-2021. They would average maybe 2k views a video(thus horrible engagement), but they would dupe many advertisers for thousands of dollars to "reach 550k followers" by peddling all sorts of useless products.

I believe this pattern is very rampant in social media and heavily manipulated, especially today for stuff nobody wants to buy but platforms are pushing consumerism/advertising. I wrote a book on how ridiculous social media is nowadays called "Enough". If anyone wants a free copy, feel free to send me an email.

thenerdhead··on Researchers identify major driver of inflammatory bowel and related diseases
Regulating your immune system can mean many things. Gabor Mate and Robert Sapolsky for example just talk about how too much stress can affect your immune system and if you have the genes, you may be more susceptible than others.

The good news of all this is that immune therapies are entering their golden age. Hopefully that means in the future you can be cured of these immune based diseases as we find the immune cell culprits like macrophages here.

thenerdhead··on A ‘plague’ comes before the fall: lessons from Roman history
I believe data is now going to be more limited:

https://www.hhs.gov/about/news/2023/05/09/fact-sheet-end-of-...

> CDC COVID-19 data surveillance has been a cornerstone of our response, and during the PHE, HHS had the authority to require lab test reporting for COVID-19. At the end of the COVID-19 PHE, HHS will no longer have this express authority to require this data from labs, which will affect the reporting of negative test results and impact the ability to calculate percent positivity for COVID-19 tests in some jurisdictions. Hospital data reporting will continue as required by the CMS conditions of participation through April 30, 2024, but reporting will be reduced from the current daily reporting to weekly.

thenerdhead··on A ‘plague’ comes before the fall: lessons from Roman history
What also comes after a plague is a period of recovery and then renaissance.

Given many world leaders faltered on their response to SCV2, society will still continue to rebuild and reimagine if it doesn't collapse!

Hopefully we can solve the chronic infection/long-term sequelae part soon.

thenerdhead··on AlphaFold 3 predicts the structure and interactions of life's molecules
A lot of accelerated article previews as of recently. Seems like humanity is making a lot of breakthroughs.

This is nothing short of amazing for all those suffering from disease.

thenerdhead··on New Strategy Could Lead to Universal, Long-Lasting Flu Shot
The novel discoveries and technology advances for medical countermeasures is impressive since the pandemic.

It is very feasible we will see this happen alongside similar developments for SCV2 and other pathogens due to the precision and potency these strategies provide for broad lineages by targeting more conserved domains of the proteins.

I am especially interested in the delivery vehicle developments(nasal, patches, etc) and platforms that have been created to adapt quickly.

Exciting times.

thenerdhead··on Software Supply Chain Security
Yep they do. Provenance and especially "build provenance" are what they are referring to but definitely misspell it.

thenerdhead··on JSR Is Not Another Package Manager
I like how they encourage best practices using a score similar to pub.dev & what npm tried to do years ago. Also the compatibility portion is quite interesting given the evolution of runtimes.

I suppose I don't quite understand the rationale of another registry. But perhaps that is what is needed nowadays with various ecosystems and their chosen defaults. What for example prevents npm from adopting ES modules tomorrow? (Although there are many opinions on CommonJS/ES Modules)

thenerdhead··on Debugging the Doctor Brain: Who's teaching doctors how to think?
There is rationale in the science article I posted above. The current hypothesis is viral reservoirs that are replication competent. Here is the blurb as well:

> Paxlovid targets virus that is rapidly replicating—which may or may not be the case for the virus lingering in Long Covid cases.

thenerdhead··on Debugging the Doctor Brain: Who's teaching doctors how to think?
Not true. There has been evidence of long term symptoms months after the first wave in 2020. By 2021 established hypotheses of viral persistence existed.

thenerdhead··on CFPB Takes Action Against Coding Boot Camp BloomTech and CEO Austen Allred
I grew up in Utah which has a certain religious culture mixed in entrepreneurship. Very good salespeople but not always morally right depending on your beliefs. The broadway show “The Book of Mormon” is a good satire as to what I’m talking about. But lived experience is better.

Also I knew of him and his early employees from his involvement in a bootcamp called DevMountain in Provo. Interesting concepts were applied there and their practices were very distasteful in my opinion such as having a previous cohort student teaching the upcoming cohort and inflating LinkedIn recommendations with the current classmates to land jobs.

Like it isn’t wrong to do, but it isn’t right either. Following Lambda over the years showed all of these patterns to larger extents.

thenerdhead··on Debugging the Doctor Brain: Who's teaching doctors how to think?
People have and have been successful. Especially repurposing HIV/AIDS drugs and ordering Paxlovid from India. Mostly due to USA prices being ridiculously high and insurance companies refusing to prescribe more than a 5 day dose.

The other major challenge is that certain approved drugs in other countries can be held by shipping due to FDA approval.

thenerdhead··on Debugging the Doctor Brain: Who's teaching doctors how to think?
In 2021 I presented my doctor a hypothesis that a COVID infection I had recently was persisting in my body leading to long term symptoms. They told me I was completely wrong and that RNA viruses do not do that.

Fast forward to today and the NIH director and leading scientists believe this has backing proof.

https://www.ucsf.edu/news/2024/03/427241/covid-19-virus-can-...

https://www.science.org/content/article/long-covid-trials-ai...

It has taken me almost three years to convince my doctor on the rationale of using therapeutics acting as COVID antivirals to treat my chronic symptoms. I now have to wait until clinical trials present data hopefully this summer to at least try.

So I hope this is a story to show that patients are helping doctors think. And it is very slow going.

thenerdhead··on Long Covid brain fog may be due to damaged blood vessels in the brain
Long COVID is horrible. I have had it for three years. I follow the research quite closely and wrote a book about what we know about it, how it affects someone and their health, and some observations on how absurd of a condition it is from the lens of someone struggling with it.

thenerdhead··on Gut bacteria–derived serotonin promotes immune tolerance in early life in mice
This might be interesting to you: https://www.acpjournals.org/doi/10.7326/M23-1831

It is suggested that many viruses can do this.

thenerdhead··on Gut bacteria–derived serotonin promotes immune tolerance in early life in mice
There are pathogens that have!

https://www.cell.com/cell/fulltext/S0092-8674(23)01034-6

thenerdhead··on Gut bacteria–derived serotonin promotes immune tolerance in early life in mice
Yes I think this pattern will get much clearer in the near future. We already see these links with chronic illness, depression, etc and gut dysbiosis. One thing I wouldn’t count out is the effect of lingering virus or viral antigens causing regular immune system stimulation leading to various unexplained illnesses to varying degrees. They also can contribute heavily to gut dysbiosis which may be linked with serotonin production and other things. Knowing how bacteria plays together is one thing but we also need to understand how pathogens can affect our guts as well!
thenerdhead··on Gut bacteria–derived serotonin promotes immune tolerance in early life in mice
What excites me the most is if we are successful with projects like https://www.humanimmunomeproject.org/ and to map our findings to the prior Human Microbiome project - https://www.hmpdacc.org/overview/ which tactful AI applications can likely find more novel findings given majority of our immune system is found in the gut. Serotonin is mostly created there, and dopamine has a strong production presence there as well.

So many great books on this recently: I contain multitudes, Immune, Song of the Cell, etc.

thenerdhead··on An ALS drug fails, again
Either you care enough to read my comment and link to comment back or you don’t. That’s called decency. Not my laziness to be quite honest with you. There’s now two comments and yet you still haven’t cared to respond about the original comment. Anyway here’s the last comment I’ll make.

This bill was written for ALS folks. It allows them to get access to safe drugs while clinical trials continue on efficacy. These provisioned drugs/illnesses like ALS, Cancer, etc provide dying people a chance to try things they would not be able to get, even through expanded access programs.

This has little to do with quacks entering the market or cynical capitalism. The drugs must demonstrate safety already and have some scientific evidence that they are on a “promising path” to efficacy while going through later phased trials.

We live in a day and age where much of our medicine is quite safe and goes through rigorous testing. But clinicians cannot get access to these drugs until they have been EUA or approved. This process takes many years to do and people suffering from these diseases have a right to try “unproven”, promising things.
