One could argue that technical debt is really just regular debt. If can see that, then you also will see that federal fiscal and monetary policy are really the long term fix, not IT departments saying “No” which of course they already do.

I’m finding twitter (now x) is rapidly replacing YouTube in direct proportion to YouTube’s increasing suckifucation. At least for new content. Mostly I use YouTube for old content that is or should be in the public domain, and for music, mostly classical which I could get from the radio for free with almost no ads. I think society really needs something like Wikipedia for video.

From the anecdotes I hear, it seems dubious from an ethical standpoint because the assertion is almost always that a person cannot think clearly due to add, so they need meds. But how can they consent to taking those meds if they can’t think clearly? Surely researching the cost/benefit and long term health outcomes of a drug like that requires the ability to focus and think clearly. It seems hard to defend, ethically.

Lastly, open office layouts suck. That’s where smalltalk happens. It doesn’t happen when everyone is in their own office.

Yep, many “just-so” stories have some aspect of truth. I feel like people citing the Peter principle as a cliche explanation for many things that are likely to be overdetermined probably correlates with intelligence, as it takes greater intelligence to consider more complicated models of career advancement.

Thanks. Yeah I guess I was thinking more about the content/research/advocacy that they themselves produce. The article you link is their news aggregator (kff health news) reposting a Sacramento Bee news article. Anyway, I appreciate you taking the time to respond. My impression that KFF doesn’t produce research or advocate for policy that is contrary to Kaiser the HMO’s interest weakly persists, but I suppose that needn’t signal any specific conflicts of interest so much as hint at incentives and business model issues. If I understood KFF’s business model better (are they a think tank?) it might resolve my concern about their alignment with big healthcare interests in general and Kaiser permanente in particular.

Seems ironic that Kaiser is banging on about the horrors of healthcare monopolies. Are they not themselves a giant healthcare monopoly that just happens to operate in a wealthier geographic area?

Sinusitis has odontogenic origins like 20% of the time iirc. I suspect a lot of heart disease results from the bifurcation of dental from general practice, eg periodontal disease being seen as a dental problem not a cardiovascular risk factor that doctors should be on the watch for.

Oh I think most people who complain are upset by the attitude of medical workers and overall hubris inherent in the profession. It’s very correlated with wealth of the patient. Wealthy people can afford well paid happy doctors and nurses, nor stressed out overworked cynical disgruntled cogs in the hmo machine. Ymmv

I wonder if the 23% were largely bots? If so that may be why the experience feels better despite the metrics? (The people I follow, Paul Graham and others, seem as active now as before. Maybe daily active users isn’t a thing to maximize?)

Thank you very much for the data point. I don’t think $150 is very much given the impact of chronic insomnia. And most of that is going to back into taxes anyway, so it’s socially beneficial.

Have you worried about dependence— an issue with any sleep med? I’ve been told daily use of edibles isn’t viable without taking days off to reset tolerance. I don’t think 20mg is very much, so maybe keeping the dose moderate and sub-recreational is key.

Quetiapine isn’t typically on the menu for mere insomnia. At least that’s what I’ve been told when I’ve asked various doctors about it. Trazodone acts an antihistamine at lower dosages as I’ve been told. My experience is that it’s much more than that, and that modern medical science is woefully unaware of what they’re doing when prescribing such drugs.

What sort of edibles and how much does such a regimen cost? I assume it’s not covered by insurance? Is your prescribed med one of the z drugs (ambien, lunesta, sonata)?

The problem is labels, and beyond that political parties. Politicians like labels because the parties depend on labels for branding. They want to use rhetoric to group people, like “poor” or “rich” and the second you do that you create the discontinuity.

Avoiding benefit cliffs requires more than understanding the issue of discontinuities, it requires the reduction of identity politics which neither party is on board with. Parties themselves are labels. They cater to the human desire to simplify and form tribes around those labels.

That was one of my first uh oh moments with gpt. Getting code that clearly had untestable/unreadable regexen, which given the source must have meant the regex were gpt generated. So much is going to go wrong, and soon.

The language changes, but more importantly students backgrounds and facility with the English language changes. Not that dumbing down the text or replacing old timey words should cost much, but cost disease and all that.
