ellisd··on Ask HN: Who is hiring? (November 2023)
Nuna (https://www.nuna.com) | San Francisco & Remote friendly (US only)| Full-time | Visa Transfer In the US, we spend an average of over $12,500 per person each year on healthcare -- that’s almost twice what other developed countries spend. Healthcare in the US costs a staggering $4 trillion dollars per year, almost 1/5 of our Nation’s entire economy. Yet with all this resource, our healthcare outcomes are poorer than other countries, people still can’t afford their healthcare, and our healthcare providers are burnt out.

Moreover, our healthcare is systemically unequal. People of color, lower income, and LGBTQ+ have demonstrably worse healthcare outcomes, a disparity grimly highlighted by the pandemic we’re living through now where people of color are three-times more likely to die from COVID-19.

How can this be, and how can we change it?

Nuna is tackling one of the most hardest problems in healthcare underlying the negative outcomes and disparities we see: how healthcare gets paid.

Today, hospitals only get paid when they do more-- more visits, more tests, more meds, more surgeries. Hopefully this helps patients get better, but regardless, the system gets paid. In fact, doing more is the only way to stay afloat.

But -- is this really the right set of incentives? Shouldn’t everyone get rewarded not just by doing more, but by when patients actually get better? Shouldn’t everyone have access to affordable, high quality care, and shouldn’t hospitals be rewarded when they deliver this care? And shouldn’t insurance companies get rewarded when they help ALL their patients get better?

Absolutely, yes. This concept is called Value-Based Care. In fact, healthcare as a whole has been trying to move in this direction for years, but making it all reality is deeply complex -- it is after all our healthcare. Nuna’s technology platform, our software apps, our vision, and our exceptionally talented team are collectively accelerating the healthcare system’s ability to make value-based care available to everyone.

In 2022, Nuna will power over $70Bn of healthcare payments for over 6.5mn patients. We also leverage our data science and platform to direct patients to the best, culturally-matched, and accessible care providers for them. Additionally, we make it transparent and easy for both hospitals and insurers to see how they are performing in value-based care by spotlighting the patients or areas where they need to pay extra attention so that they can provide good care to all their patients and get rewarded.

Nuna is unique - we have brought together an exceptional team of over 200 people. We are the industry’s best in healthcare data, analytics, engineering, clinicians, and value based healthcare experts. We have joined forces to create a more equitable health system for everyone.

Our dreams and ambitions to change healthcare as we know it are big. If yours are too, we want to work with you.

ellisd··on Oregon decriminalized hard drugs – early results aren’t encouraging
This is a fascinating view on the sedative crisis. The lack of shame from "normal" people feels very real. Complete inaction to the staggering and ever increasing death count rate and bodies piling on the ground IS utterly shameful, but not for 2023 USA.

I've noticed as well that when I've spoke to friends and family about open drug use and markets I become the one who's "out of line."

Do we as a free society have no shame left to express?

ellisd··on Ask HN: Who is hiring? (August 2023)
Thank you for the feedback, I've adjusted the post to make this clearer.

Nuna's HQ is located SF but we are a hybrid and very remote friendly workforce. You must be located physically in the United States however to work.

ellisd··on Beautifying our UI: Giving Gitlab build features a fresh look
Dear Gitlab... please focus on finishing out your parent/child pipeline concepts. I'm very disappointed to see basic CI capabilities missing in 2023. None of the shiny AI things matter if CI isn't solid.

Today parent pipelines cannot consume any test report from the child pipeline using the Gitlab CI DSL: https://gitlab.com/groups/gitlab-org/-/epics/8205

We would effectively need to roll up the child pipeline's results manually in the parent pipeline by calling APIs ourselves.

If yours are too, we want to work with you.

ellisd··on Analysis of energy consumption of Bluetooth Low Energy versus Bluetooth Classic
It's a really good point, a wire based sensor eliminates a dead battery and interference as failure points. The Wireless accelerometer feature should come after the basic animation concepts have been ironed out and tested. I really appreciate the reference to RMF* type radios - I had not come across them in my reading.
ellisd··on Analysis of energy consumption of Bluetooth Low Energy versus Bluetooth Classic
I'm actively trying to avoid a phone, rather a dedicated LED controller likely containing an AVR or ESP32. A simple button based paring process to associate the wrist band with its personal controller is likely necessary to solve the RSSI issue you point out.
ellisd··on Analysis of energy consumption of Bluetooth Low Energy versus Bluetooth Classic
Off stage, within the 2.4 ghz ocean: the audience. Your point is well taken that spectrum saturation will be an issue. Low range/power consumption, not centralization are the driving factors in the design. Thanks for the share!
ellisd··on Analysis of energy consumption of Bluetooth Low Energy versus Bluetooth Classic
On this topic: I'm very interested in streaming accelerometer data in real time from a wrist worn device to a body mounted LED controller. My goal is to modify the LEDs animations based on the hand position. Given the sensor bitrate and low latency required for this data, it would appears that BT Classic may be better solution than BLE. Beyond the power consumption needs, BLE connectionless broadcasting of the data seems easier to handle with than having to manage pairing two device. You could simply pick the strongest signal and be reasonable assured that it's your personally worn wrist device.

Am I missing something?

Fun Fact: COBRA coverage doesn’t exist when your startup shuts down and you’re laid off.

This did not stop my insurance company from cashing my premium payments for over three months, even though they silently canceled my policy (no email, mail, or phone call). I only learned I had no coverage when I actually tried to have a dental appointment. Refunds of these premium payments took another another 3 months to be returned!
