As an active mushroom hunter (born in Russia), can confirm that this is extremely unlikely and uncommon. People who have done this since birth know how distinguish between mushrooms, and usually stick to the variety that they love the best. In Russia, the mushroom of choice is the "White Mushroom" (or Porcini, King Bolete) or Rejik (Milky cap) and Chanterreles, and a few others, and that's it, with the top 2 being the most sought after.

I've been doing this for over 35 years, and always go after the same mushrooms, and have never been poisoned. So, very unlikely someone much older would make a mistake, sounds like foul play.

I had two friends that were killed in a car crash inside city limits. One guy was turning left, and the other guy going 90 miles an hour in a 40 mile zone. Happened around midnight. Hit them perpendicular, and killed two friends and critically injured driver who survived and is recovering.

Have become much more sensitive about car crashes and speeding as a result obviously. Forces you to re-assess your life.

Realistic solution - Install speeding cameras everywhere, and most intersections. And levy heavy fines if a person is speeding 20 miles above speed limit, possibly even revoking driver's license in that case.

I remember trying to hire an iOS developer on Upwork, and got an American developer to bid on the project that seemed too good to be true. Turned out he was, and it was a fake profile. Figured it out only after agreeing on payment and about to start work, before realizing something was off. Contacted Upwork and they took profile down.
