I don’t care about milk, but in general we don’t want the health care system swamped by preventable things on a large scale. This an issue in Covid that was impossible to hammer into the freedom nuts and current measles fans. I say this as a physician who worked covid wards. Here there were times when there were no more icu beds or ventilators available and people with unrelated medical issues were badly affected by that.
You can make an argument against me by extending it to saying let’s ban antifreeze or whatever by my logic but that isn’t usually something that scales the same way because it’s a relatively rare but serious event someone drinks it compared to public health measures in a population level with a small change to a high frequency but variable event rate.
I think he is trying to set us up and also occupy the news cycle to distract. We retaliate with tariffs, he blames us for his domestic economic woes and increases tariffs, we retaliate with electricity and potash for leverage, public support for Canada slowly drops. Thus he slowly moves the goal posts eventually to seize these resources because they are of national importance. A stretch? Maybe but look at Panama, Greenland, Ukraine minerals actions.
There is no going back to the previous relationship because Canadians feel betrayed after years of big sibling adoration and blame pre-existing, worsening unrelated domestic issues on Trump’s actions. My honest take of public sentiment is it has taken such a dive at this point if 9/11 happened in this era the passengers landing in Canada could camp out on the runway huddling for warmth by barrels until busses home are arranged.
Can I ask an adjacent question? I have a bunh of DNS A name entries for locallyaccessedservice.mydomain.tld point to my 10.0.0.x NAS's nginx reverse proxy so I can use HTTPS and DNS to access them locally and via Tailscale. My cert is for *.domain.tld. It's nothing critical and only accessible within my LAN, but is there any reason I shouldn't be doing this from a security point of view? I guess someone could phish that to another globally accessible server if DNS changed and I wouldn't notice but I don't see how that would be an issue. There are a couple nginx services exposed to public but not those specific domains so I guess that is an attack vector since.
AI is an assistive tool at best but it can probably speed up by reflowing text. I use dragon dictation with one of the Philips microphones and it makes enough mistakes that I would probably spend the same time editing/proofing. Had a good example yesterday where it missed a key NOT in an impression.
As aside, the after work is what burns out physicians. There is time after the visit to do a note, 5 min for a very simple is reasonable to create dictate fax do the work flow for billing and request a follow up within a given system. A new consult might take 10 min between visits if you have time.
For after hours, ER is in my opinion a bad example because when you are done, you are done.
Take a chronic disease speciality or GP and it is hours of paperwork after clinic to finish notes (worse if teaching students), triage referrals, deal with patient phone calls that came in, deal with results and act in them, read faxes etc. I saw my last patient ~430 yesterday and left for home at 7 dealing with notes and stuff that came in since Thursday night.
I happily used Migadu but switched to iCloud. The bus factor for my family is 1 with technical knowledge whereas with domains paid up for 10 years and iCloud family it is higher.
As a Canadian end user with a nexus card, it just works, and it works with less errors and issues than my revenue canada login which has poor UI and steers me towards sign in partners with by making that the natural login with dark patterns. Come to think of it, I'm surprised the nexus program hasn't been cancelled yet.
For sure. I hope it shows nothing and it is a one off but would not want to miss something that needs treatment or predisposes you to recurrence. Take care!
A lot of drugs and efforts to treat sepsis/SIRS with inflammation have fizzled in trial. Tocilizumab, JAK inhibitors, and particaularly dexamethasone with its low NNT in COVID were interesting ones and suggest there is something to it but a complicated issue.
Glad you are OK. I am not your internist (/IANAL disclaimer) and I usually don't comment in HN medical threads as weird opinions here, but reading HN taking a break from my work, I'll say generally unless you have some reason like a recent dental procedure to have gotten it depending on your age I'd ask to get a (non urgent) colonoscopy or at least MR enterography or at least consideration of it with a reason why not with someone who knows the particulars of your case and can give you real medical advice.
CT has limited ability to pick up GI malignancy that can lead to translocation of bacteria from colon. If you had something like an autoimmune colitis that lead to translocation you would need to get treated and not all have preceding symptoms.
I’m lucky, I have a tube 2019 running jellyfin into a 5.1.2 setup through my avr and DTS/atmos + HDR10/DV work with jellyfin. I switched from kodi a few years. Do you have Dolby processing and up mix 2.0 turned off in the shield settings? Those cause stutters and some issues for me.
It’s great. A stock Nvidia shield 2019 tube (!) plays 4K HDR/DV DTS content without stutter or issues with passthrough an AVR which is impressive. I can't say Kodi and various forks were able to do that, at least in the past. Lots of griping about the tube in general as well but it manages without transcoding.
Re: Quebec hydro, I can’t think of an example of a crown corp or asset (see 407) being sold in Canada that has benefited the public with better quality at a lower price in the end. Privatization drives the need for shareholder profit to extract value more than efficiency so gains aren’t passed on. People contort themselves to think this isn’t the case… but the track record for power, automotive insurance, cellular service says otherwise… The only good argument I’ve ever seen of outside of pretending efficiency leads back to consumers is hidden externalities being covered by the government which is fair toy stability of a public service, averaging out costs for really isolated areas, and decent jobs with pensions is worth something too.
That's cool and thanks for sharing! If you are looking for future direction generating reports by category and overall and ability to add attachment of PDF/photo receipts to individual items is really helpful. I use these type of tools to keep track of business expenses for my corp (employee n=me) for my accountant come tax time and have never found the right tool for it. Tried even abusing invoiceninja, craterapp, a few others outside their use case. Eventually I settled on paperless-ngx (pulls from scanner, email address off phone) manual entry into excel now seatable for this on this but still not ideal. Firefly III seemed useful but daunting. I don't need double ended accounting or care about budgeting where I spend money... just receipt, table, report, print all and end of year.
The general population is not able to grasp broader issues like COVID 19 response, climate change, addressing inequality and systemic barriers to health/employment/life that don't directly impact themselves, or understand that you can't just hit spokes with a wrench when it comes to the government without the system falling apart anymore than my cat can plan ahead to get dinner. And I am not saying I am smarter in areas outside my lane and also get manipulated but there is no uncertainty he, were he ANYONE else, would be guilty of at least some of what has been investigates (Jan 6, hush money to say least) which has been pushed away for political reasons by those propping him up. What you say just shows people can be manipulated by media, group think, religion and oligarchs into voting against their own interests yet again and zero, truly zero pity that hurt themselves supporting him. Us Canadians have our own populists too on the right working against my values (Smith, PP, Ford) but at least they aren't criminals out for revenge.
I don't think it's likely to happen but that is what I also said about a tariff trade war with Canada until recently. The actions here aren't rationale.
I think that would just be a pretext to annex Canada much like civil disobedience in the US would be a pretext for martial law and entrenching his position. As a Canadian: nothing polite to say about the USA going forward.
Hopefully they also charge my government a 25% tariff for the humanitarian aid delivered by the plane a US resident damaged. /s Good grief. That must be expensive to fix.
I’d be curious re your thoughts on something like a iPhone 16 vs my d6 MkII with a 24-70 or 200 EF lens. The telephoto obviously wins at distance but for family portraits I’ve found I get better photos with the phone and I’m not sure why. I chalk it up to iPhone image processing voodoo vs my setting choice and raw editing skills.
I dumped a decade of android after the pixel 6 and 7 due to the pathetic camera. It couldn’t take an picture of a flat piece of paper without distortion!
This is an open access journal by two people whose publication history you can look up if you want to draw your own conclusions. Read the disclaimer at the top of the link. Don’t bootstrap its credibility by linking it to being at NIH (which does mean something) anymore than saying something found on Google is from the company itself.
The wealth is also generated by multiplying purchasing power by leveraging against an asset with a mortgage (e.g. when else does a regular person get a $XXXk loan).
Joy, some selfish git endangering others by asserting his right to do whatever. I appreciate it's not going to be a particularly well received comment, but as a Canadian I've been reflecting on Gander and US/Canadian relations and empathy in the past while reading the news lately. And we aren't there anymore. They should just send these two planes and our firefighters back given all the rhetoric and threats of tariffs thrown at Canada lately.
I look at it a bit directly. I shoot with a canon d6 mark 2 dslr regularly on trips up north and hiking and have spent way too much money on this stuff. While the dslr with a 200-400mm lens is superior for stuff like polar bears (yellowish white on usually low contrast backgrounds) at a safe distance, my iPhone 16 can in a single shot get a great family photo or wide angle nature shot thanks to whatever optics and processing magic it has. For similar I’d have to expend a lot of effort with the dslr and raw editing after.
One other consideration is I usually give old tablets, phones, laptops to my extended family. In this case I won't as not enough parts. I think when I do replace the motherboard it I'll the old mobo/ram/ssd as a blueiris machine instead of running it on a VM.
