There is demand for a lot of things in a lot of areas, but it isn't allowed because it would be for the benefit of a select few at the cost of the many. Casinos, bars, hotels -- any kind of business, really, aren't allowed everywhere even though there may be demand for those services from people who don't actually have to live in that area.

BMI is a tool for doctors to use, along with clinical judgement. No doctor is going to look at a lean, well muscled individual and tell them they're at risk for heart disease because their BMI is high.

BMI is something that fails in theory and works in practice.

Japan had 120 million people in the 80's. China is currently at 1.35 billion. There were a million new millionaires created in China in 2014. I'm not sure that the Japan situation compares to China very well.
