Some Japanese toilets have a system to create negative pressure so that no aerosols escape even when the lid is down.
AFAIK, shower heads are also problematic. I recall NHS conducted a study and they concluded many vulnerable patients get infected by aerosols during showering. Shower heads that are not cleaned periodically get very nasty biofilm buildup inside the head.
I think that as a consequence some NHS trusts switched to some showerheads that get cleaned and rotated periodically.
