I'm not 100% certain but it seems like this may be an example of severe gravitational lensing, causing the same galaxy to appear as multiple images in different periods of time

There's no strong lensing there. If the background galaxies were lensed enough to have multiple images they would also be incredibly distorted.

But that does look like a small group/cluster.
