The concept that as a user, I don't know the ultimate bribe cost. Plus constant worry that it may go up or worse, the rules have changed and prices( read bribes ) went up. What a backwards way of shimmying your way into what should be : consumer + producer relationship.
I want to live in a reality where I know your costs and you tell me how much divided amongst your users is my share to pay. Let's get Google out of the equation.
What if we had mandatory HTTP headers: "Content-Cost: 0.02" and my browser had a credit card linked against it and I will decide if your content is good enough to buy.

While I am not an expert,- this is evident from what I read about on the Fukushima topic in the past written by other investigators. From what I remember, the lost cooling capacity caused by overwhelmed backup system could have been prevented if the rushing water could not reach it. I think it was GE and whichever other participating bodies there at the time that made the engineering and budgeting decisions to design it that way. In an earthquake prone zone, with danger of tsunamis no less.

I think that if you can update a user's password without them inputting, then you have two problems.

edit: seems that comments suggest doing the md5(bcrypt(hash)) can be used to upgrade across the board
edit: I don't know if this came up before, but based on how they stupidly tried to cache the login session tokens with md5, instead of running through the 12 work factor bcrypt, I can assume that they saw this as a bottleneck.
Instead of dropping the work factor or doing this caching baloney, could a service be made that runs on extravagantly fast hardware, which provides an API for strong, high work factor bcrypt, pbkdf2 based authentication.
I can assume that at around 10 rounds, each attempt takes about 50 - 100 millis

Can you elaborate on peer review and code review ? I have worked with and without these steps and can quantify the hours required and the quality shift in having these steps.
Do you think that these two can be disconnected from the development workflow with acceptable consequences? Or is it that there might be other standards or development measures which are no being met, such that there IS a need for review ?
