I'm a physician. I love the value I bring to patients, and I enjoy what I do (most of the time). But the industry is disgusting. The people who are creating the real value for patients (nurses, technicians and doctors) have been saddled with increasing regulations and there are incredibly levels of administrators and belt-phone business guys riding them to extract every profit out of the environment.

There is no chance I would do it again.

I can't speak for law school.

Medical school is absolutely not career training. It simply gives you the knowledge and vocabulary so that you can be trained to be doctor. The additional training, of course, happens in your residency.

You are utterly unprepared to truly be a physician when you graduate medical school, even if you have earned the right to be called 'doctor.'

These are generally fair better behaved than the more common and far more deadly type of cancer you are probably familiar with, pancreatic ductal adenocarcinoma. While pancreatic ductal adenocarcinomas have a dismal prognosis, most neuroendocrine tumors have a good survivability index.

For node + browser, I'm partial to substack's tape module[1][2] to output tap. Works great with browserify too.

That is a code smell. node and require works really great with small modules. npm link in development, tarball up and scp to your box for deployment.

You probably shouldn't be using npm for production builds. npm is good for libraries, but applications should probably be tarballed.

If find it's easier to distinguish modules from files by requiring modules like:

  require('foo');
and application files like:

  require('./foo.js'); //.js extension

We've been working on a similar problem. Writing in markdown, pre-processing the saved markdown to truncate to 80 column (best fit) paragraphs and then diffing on the markdown.
