Ask HN: Easy to use service to handle applications

This is for an academic institution which needs to receive applications for eg scholarships, conferences, etc. Forms need to handle typical personal data, accept documents, send confirmations, store everything online, and provide a simple front-end to query all the information stored. Happy to install it on own server but would prefer something maintained and hosted by a third party so they control security, etc.
