Always personality and communication problems. You can find a fairly large body of writing on this topic. I’d start with Fred Brooks’ The Mythical Man Month and Gerry Weinberg’s The Psychology of Computer Programming.

Old people not listening to young people

Young people who think they already know everything better than their seniors :)
