It was and still is defective, and it always will be until it includes real time road and traffic data, and that requires installing sensors in and along roads.

Elon may be the wealthiest man on the planet but it would take a lot more than his last dollar to implement even a few percent of the cost to do that, and it still won't be safer than an attentive driver.

I've been flamed here over the past few years for pointing that out but as time passes Tesla owners keep providing the evidence that proves it.
