Every place I've ever worked with a 90 day password rotation policy I've fought hard against them. I've pulled up papers and filed tickets and whatever else I could do to eliminate the scourge that is frequent password changes.
My last employer recognized the flaws in our password policy but still required them for compliance reasons.
