One example from my anecdotes is when they did consulting work for a telecom in my home country and pushed for firing half the technicians who were working in pairs. One of the reasons they were working in pairs was safety, like one holding the ladder and they used this as an argument and said they can just be replaced by a safer ladder. Said and done.
