Not vaccinating people and destroying the healthcare system is certainly one way to get a population pyramid that looks like Haiti's. Those who remain alive will have lower probability of dying due to diseases like COVID 19 that hit the elderly the hardest and lower probability of having their deaths accurately recorded.
