Not all. I once went with a coworker for lunch at Del Taco. He ordered what turned out to be on our place-mat. I was astounded that it looked identical to the place-mat photo, if anything it looked better than the photo.

If they’re saying “fresh” and they’re serving frozen pucks, you definitely can go after them. Juicy, probably not. Tall (unpressed sandwich) isn’t actionable but missing ingredients and uncooked patty weight are.

Depends on the brand, there are a few around here that aren't so bad. My last (and absolutely final) trip to Stake and Shake, however, their burger patties were so thin you could actually see through them.
