So where's the money coming from then?

Insurance? Government subsidy?

Remind me again, who pays for insurance and government? Or are these entities just expected to absorb the difference out of the goodness of their hearts?

It's insurance, which is why premiums could go up. When medical providers raise rates that insurance gets charged, people get charged more for insurance. Simple as that, we eat the cost but it's obfuscated
