where i work, alot of this could be fixed by recording/watching what users are actually doing then improving the UI so its more obvious what is supposed to be done.

its like the old story about how on a college campus some administrator put up dozens of signs about not walking on the grass between buildings, put up mini fences around the grass, etc etc, eventually someone just built a sidewalk where the students were walking.
