1) How often do alarming system failures (as Rube Goldberg-esk as these systems can be) result in actual deaths? In this case multiple layers of alarm systems failed simultaneously - leaving 2 children without parents.
2) The conclusions ultimately questioned the need for the structure that housed this equipment. That struck me as quite a solution - and probably why you see so much heavy industry outside in the elements.
