Ask HN: How to design development processes and policies?

The startup I'm a part of is beginning to grow past the point that our ad hoc development processes (e.g. how we review our code, how to distribute tasks, etc.) are no longer viable.

What resources (books especially) would you recommend to learn to formalize processes and policy that would survive scaling from less than five people to a hundred?
