Ask HN: Open-source, self-hosted alternative to Typeform – Would you use it?

Typeform (https://www.typeform.com/) is a great service, but slightly expensive, especially when you want logic jumps and other features. Would you use an open-source, self-hosted alternative? Maybe something like pay once (around $40 or whatever), and get it for yourself, instead of paying monthly.
