Ask HN: What are some mechanisms you use to deliver things as a team?

I work on a team of about 12 people and we have a well defined roadmap. However, whatever processes we follow—sprints, daily scrum etc., don't seem to contribute much towards advancing the roadmap effectively. What are some processes/mechanisms that you follow to be effective in terms of just completing the tasks that you set to complete over weeks/months?
