Ask HN: Previous HN post about incident or catastrophe management in hospitals?

Within the past couple years, I read a PDF written by a doctor who outlined various principles for reducing catastrophes within a hospital setting. There were many parallels to engineering and I found the document enlightening. I can't find it now and I'm wondering if anybody remembers it or has it handy. Much appreciated!
