Ask HN: How can one get a debit card issued in a specific country?

Some services restrict a range of countries they accept payments from. Is there an easy way to issue a debit/prepaid card from, say, EU country for a US resident and vice versa?

I've spent the whole day searching for a solution and found basically nothing.
