Ask HN: How do you manage database updates?

I run a SaaS website with a sharded cluster of MySQL databases. They work well, but updating the schema is always a bit of a hassle, as is rolling back changes in the case of a regression. So I was wondering, what strategy do you use to manage your database schema updates and how do you roll things back, especially when the number of database instances is greater than 10?
