The only way to avoid ID theft is to lock your credit with the big three credit reporting agencies. With the number of breaches occurring, you can bet your data is in the hands of at least one fraudster. This adds inconvenience when you are applying for credit, however it's what all the big celebrities do.
Credit monitoring is a good idea too, but that's more for damage control than prevention.
You can't avoid identity theft, because you not in control of all your data.
Anthem/OPM/lastpass will get breached or Experian will sell your data, etc. etc.
The only thing you can do - harden each line of defense available [1].
